New Client Questionnaire

To submit a new client questionnaire please complete the guide materials first, then complete the questionnaire
 
If you don’t hear from us within a week please touch base  because occasionally emails are redirected to spam for reasons known only to the universe.    If it has been more than a year since you submitted a questionnaire please update it and resend.  

If you are an established client and it‘s been a few years since you got your last dog please go ahead and resubmit this questionnaire. 

If we determine that one of our puppies is a good fit for your overall situation and needs we will send you another questionnaire with questions about color, gender, and ask for more information about your lifestyle for us to go over with you during a phone interview.  During the phone interview you can ask us any questions you may have, we can tell you more about our dogs, schedule a visit, and we can work together to determine what litter and puppy would best fit your needs and preferences.
